At Alliance Health Centers, our goal is to provide education and help manage your health at all stages of your life.
Primary Medical
At Alliance Health Centers, we offer high quality primary medical care to all ages. Our providers deliver care that is focused on the patient’s specific needs. Primary medical care services include: preventative health, chronic disease management, sick care, well-child care, well-woman exams, pregnancy diagnosis, immunizations (for all ages), geriatric care, physicals, health education, laboratory testing (including blood draws on-site) and coordinated referrals for specialty care.
OB/GYN
OB/GYN services include prenatal and post-partum care for pregnancy. General gynecological care, including minor procedures and screenings, are offered as well.
Behavioral Health
Behavioral health services include assessment and consultation with a psychiatric nurse practitioner to determine appropriateness for medication and/or with a licensed clinical social worker for therapy or counseling services. Initial appointments include gathering information about your symptoms, history, and lifestyle as well as setting goals for treatment.
Providers can see individuals 6 and older. Minors must have parent or legal guardian present unless otherwise stated. For desired services not available at our location, referrals may be available. You can self-refer for behavioral health services, some insurances require a provider referral for therapy.

Enrollment Services
A certified Indiana Navigator is available who has met training requirements to help you and your family apply for Medicaid. Our navigator can help you if you are uninsured, looking to explore insurance options or need help understanding your current insurance coverage. This service is FREE. You are not required to be a current patient at Alliance Health Centers to meet with our navigator. Talk to an Alliance Health Centers’ team member or call 260-266-0780 (Option 6) to schedule a free appointment with our navigator.
